Passages Hospice to offer advanced directives information on April 16th
4/14/2011 - Chicago, Illinois - Passages Hospice will hold a public information session to highlight the importance of advance healthcare decision-making on April 16 as part of National Healthcare Decisions Day (NHDD).
Everyone is invited to find information and tools to talk about their end-of-life wishes with family, friends and healthcare providers, and execute written advance directives (healthcare power of attorney and a living will) in accordance with Illinois state laws.
The information session will be held at Jackson Park Hospital, 7531 S. Stony Island Avenue, on Saturday, April 16th from 9:00am to 12:00pm.
“By completing an advanced directive, families and healthcare providers will be more able to honor patient wishes in a crisis,” said Seth Gillman, Administrator of Passages Hospice.
